These Terms of Service govern your use of equinoxhealthmd.com and the services provided by Equinox Integrative Health (DBA name), a trade name of Equinox Integrative Wellness (legal entity), operated by Dr. Allison McClelland, M.D. (collectively “Practice,” “we,” “us,” or “our”). By using our website or booking an appointment, you agree to these Terms.
Equinox Integrative Health provides integrative health services including medical acupuncture, pediatric acupuncture, Korean hand acupuncture, cupping therapy, Gua Sha, and telemedicine consultations for established patients. All services are provided by or under the supervision of Dr. Allison McClelland, M.D.
Our services complement, but do not replace, conventional medical care. If you are experiencing a medical emergency, call 911 immediately. Nothing on this website constitutes medical advice. Always consult a qualified healthcare provider regarding your specific health conditions.
By booking an appointment you agree to: provide accurate and complete information; that you are booking for yourself or, for minor patients, as a parent or legal guardian; and that payment is required to confirm your appointment. We accept all major credit and debit cards.
We request 24 hours’ notice for cancellations or rescheduling.

Telemedicine consultations are available exclusively to established patients who have completed a prior in-person initial consultation at Equinox Integrative Health. By booking a telemedicine session you confirm you are an established patient and consent to the telehealth format.
A parent or legal guardian must be present for all appointments involving patients under 18. By booking a pediatric appointment, you confirm you are the parent or legal guardian and consent to treatment on their behalf.
Age Restriction: SMS messaging services are intended for users 18 years of age or older. By opting in to receive SMS messages, you confirm you are at least 18 years of age. For minor patients, a parent or legal guardian must provide SMS consent on their behalf.
If you opt in to SMS communications by selecting the optional consent checkbox on our booking form, you agree to receive SMS text messages from Equinox Integrative Health for appointment reminders, confirmations, scheduling updates, and follow-up communications. Message frequency varies. Message and data rates may apply. For questions about your text or data plan, contact your wireless provider.
Opt-Out: Reply STOP at any time to cancel. After sending STOP, you will receive a one-time confirmation message confirming your unsubscribe. No further messages will be sent.
Rejoin: To resubscribe, reply START or opt in again through our booking form.

Carrier Liability: Carriers are not liable for delayed or undelivered messages.

No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. Information sharing to subcontractors in support services, such as customer service, is permitted. All other use case categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
All services are payable at time of booking or service. We accept all major credit and debit cards. Payments are processed securely by Stripe, Inc. The price displayed at time of booking applies to your appointment.
To the fullest extent permitted by applicable law, Equinox Integrative Health and Dr. Allison McClelland shall not be liable for any indirect, incidental, special, or consequential damages arising from use of our services or website.
These Terms are governed by the laws of the State of Arizona. Disputes shall be resolved in the courts of Maricopa County, Arizona.
We reserve the right to update these Terms at any time. Continued use of our website or services constitutes acceptance of the updated Terms.
